Street Listing Notice:

The 2012 Annual Street Listing forms have been mailed to every known household in the Town of Charlemont. Please update the information if necessary, sign, and return the form in the self addressed envelope provided. If you have not received a form please contact the Town Clerk.

If the form is not returned, you may be removed from the active voting list.

Licenses:

DOG LICENSES

All dogs residing in Charlemont must be licensed with the Town. 2012 licenses are available from the Town Clerk during office hours or by mailing the dog license application below, a copy of your dog's current rabies certificate, proof of spay or neuter, and a self addressed stamped envelope with the appropriate fee to the Town Clerk.

FEES:               Male or female                                  $10.00 each

                         Spayed or neutered male or female      $5.00 each

                        Call for kennel license fees

A late fee of $10.00 will be added for licenses issued after March 31, 2012.

• Selectman –3 year term
• Moderator –1 year term
• Assessor –3 year term
• Charlemont School Committee –3-year term
• Mohawk School Committee –3 year term
• Tree Warden –1year term
• Planning Board –3 year term
• Associate Planning Board –1 year term
• 2 positions on the Board of Health each with a 3 year term
• Tyler Memorial Library Trustee –3 year term
• Parks & Recreation Commission –5 year term
• Constable –3 year term

The last day to register to be able to vote in the Local Elections and Town Meeting is Wednesday, May 8, 2013
